Documented Work Experience

Various documents must be submitted in order for you to be eligible to receive credit for Work Experience. Please review the following and submit when necessary.

• Employment Verification Form
• Verification of Surgical Experience
• Surgical Log Book or an authorized letter from your medical facility that attests to completion of 125 scrub procedures
